  2. Your Social Insurance Number (SIN) - for tax purposes.

Personal Information contained in this form is collected under the authority of Municipal Act, 2001, S.O. 2001, c. 25, s. 227(c) and will be used only to ascertain eligibility for a Bursary/Scholarship.
